Re-Enrollment Event!

Location: School's computer lab
Snacks will be provided!
Please bring the following items with you:
~ $25.00 Application Fee (non-refundable)
~ Current Proof of Residency* (This is a new annual requirement placed on community schools from the Ohio Department of Education.)
*A valid proof of residency includes evidence of your current street address dated within 30 days of presentation. Ex: voter registration card, leases, mortgage statement, utility bills, bank statements, rent receipt.
*Please be reminded that any time you make a residency change the school must be notified and we must receive proof of your new address. Parents/guardians are also responsible for informing the school of any changes to the legal custody or guardianship of their child.
Please note to be considered fully re-enrolled in our school you must provide the above items to the school office.
